Please use the Controlled Chain of Custody form for all Cannabis/hemp/mushroom samples. This must be filled out, emailed to the lab for approval and signature, and the signed copy submitted alongside the sample.
For all other substances, please use the Chain of Custody form or the General Chain of Custody for submissions with multiple samples.
