Confidentiality
All client results and personal information collected upon sample submission are maintained in strict confidence. Access to client records is restricted to employees of MB Laboratories Ltd. Reports or copies of the results
will only be released to the client or to an agent or party designated specifically by the client in writing.
Information Collected
Client name, mailing address and telephone number are collected to correctly identify and communicate with the client. Facsimile numbers and/or email address are used to communicate analytical results back to the specific client.
Data is entered into an electronic laboratory management system which will store the information in a unique, numbered file (called Job #). Final results are
printed onto a three-part NCR form. The top (white) copy is mailed to the client;
the middle (yellow) copy remains with the financial records of the company
(kept for 5 years) and the bottom (pink) copy is kept a minimum of 5 years as a numerical record of the results. An electronic copy of the analytical report is kept on file at one computer station and on an archival back-up data storage device.
The electronic copies are kept isolated from outside contact by 2 sets of firewalls
and the archives are removed from all inter and intra computer contact after back-ups are complete.
MB Laboratories Ltd does not trade, share or sell client information.
Accuracy
The laboratory will accept changes in information and sample description until
a final, signed copy has been produced. An information error made by the laboratory will be corrected with a new amended analytical report. An error made by the client can not be changed once the final report has been generated.
